Psychosocial Rehabilitation

Psychosocial Rehabilitation (PSR) services are designed to help adults with psychiatric disabilities increase their functioning so that they can be successful and satisfied in the environments of their choice with the least amount of ongoing professional intervention. PSR focuses on skill and resource development related to life in the community and to increase the member’s ability to live as independently as possible, to manage their illness and lives with as little professional intervention as possible, and to participate in community opportunities related to functional, social, educational, and vocational goals.

The service is based on the principles of recovery, including equipping members with skills, emphasizing self-determination, and using natural and community supports. The program is offered 5 days a week.

The program includes skill development, educational, daily living activities such as Life Skills (Development and maintenance of daily living skills; Wellness development and recovery; Social, relationship, recreational building skills; housekeeping, goal setting, shopping for necessities, nutrition, food planning, preparation, and cooking, use of transportation facilities, money management); personal care (health care, medication self-management, hygiene, grooming, social and community skills; use of leisure time; educational activities (e.g. assisting individual to participate in adult education or special interest courses); and pre-vocational activities focused on development of positive work habits; employment development; and GED assistance. Our program equips individuals with real-world skills using natural and community supports while providing individualized interventions.

Outpatient Therapy (OPT)

Outpatient Therapy provides psychiatric, counseling, and support services. The goal of the services is to assist individuals in the improving opportunity to be successful in family, educational, occupational and social settings. Our staff specialists are available to work with children, adolescents, adults, and older adults to enhance an individual’s natural support system while actively involving the person in all aspects of treatment. Specific Features of the Program Include:

Behavioral Therapy, Individual Therapy, Family Therapy, Marital and Couples Counseling, Group Therapy, Anger Management, Recovery-focused Care, Anxiety, Bullying, Depression, Posttraumatic Stress Disorder (PTSD), Sexuality Identity, Loss / Crisis / Grief / Trauma

We offer a variety of services to meet the needs of our community, including individual, couple, family and group counseling. Day, evening, and weekend appointments are available. Every effort is made to find the therapist most suited to the client’s needs.

Medication Management (MM)

Pharmacotherapy (medication management) services to include evaluation and prescribing of medications that adhere to local, State, and federal guidelines. Additionally, the qualified medical professional (MD) will document all medical orders and prescriptions, as well as facilitate written informed consent from the consumer for each medication prescribed. Individuals with behavioral health challenges such as ADHD, depression, anxiety or trauma, medications may be used as one part of their individual treatment plan. They are used to complement other treatments that the individual is receiving, such as Outpatient Therapy, to reduce symptoms related to behavioral challenges.

Before recommending medications, we consider the diagnosis as well as their unique needs and goals for treatment. We also look at their overall medical history to learn whether prescription or over-the-counter medications may reduce behavioral symptoms. Medications may also be recommended to address medical problems that may be making it harder for an individual to move forward in their treatment.

Medications are prescribed by one of our physicians and reviewed continually to make sure they are as effective as possible. When medications are used, our medication management staff work closely with the outpatient therapists, physicians, parents and others involved in their case management team to ensure that they are used correctly and meet the individual’s needs.

Community Support Team

Community Support Team (CST) services consist of community-based mental health and substance abuse rehabilitation services and necessary supports provided through a team approach to assist adults* in achieving rehabilitative and recovery goals. It is intended for individuals with mental illness, substance use disorders, or both who have complex and extensive treatment needs. The individual’s clinical needs are evidenced by the presence of a diagnosable mental illness, substance use disorder (as defined by the DSM-5, or any subsequent editions of this reference material), or both, with symptoms and effects documented in the comprehensive clinical assessment and the PCP.

*Note: The age at which a beneficiary is considered an “adult” is determined by the funding source. Medicaid-funded services for adults begin at age 21. Medicaid beneficiaries under the age of 21 may be eligible for adult services under EPSDT.

This is an intensive community-based rehabilitation team service that provides direct treatment and restorative interventions as well as case management. CST is designed to:

  • Reduce presenting psychiatric or substance use disorder symptoms and promote symptom stability;
  • Restore the beneficiary’s community living and interpersonal skills;
  • Provide first responder intervention to deescalate the current crisis; and
  • Ensure linkage to community services and resources.

This team service includes a variety of interventions that are available 24 hours a day, 7 days a week, 365 days a year, and are delivered by the CST staff, who maintain contact and intervene as one organizational unit. CST services are provided through a team approach; however, discrete interventions may be delivered by any one or more team members as clinically indicated. Not all team members are required to provide direct intervention to each beneficiary on the caseload. The team leader must provide direct clinical interventions with each beneficiary. The team approach involves structured, face-to-face, scheduled therapeutic interventions to provide support and guidance in all areas of functioning in life domains: emotional, social, safety, housing, medical and health, educational, vocational, and legal.

The CST licensed professional or associate level licensed professional team leader drives the delivery of this rehabilitative service. In partnership with the beneficiary, the assigned CST QP identified as the person responsible for the PCP has ongoing clinical responsibility for developing and revising the PCP.

Under the direction of the team leader, CST services are delivered to beneficiaries, with a team approach, primarily in their living environments and include, but are not limited to, the following interventions as clinically indicated:

  • Individual therapy;
  • Behavioral interventions such as modeling, behavior modification, behavior rehearsal;
  • Substance use disorder treatment interventions;
  • Development of relapse prevention and disease management strategies to support recovery;
  • Psychoeducation for the beneficiary, families, caregivers, or other individuals involved with the beneficiary about the beneficiary’s diagnosis, symptoms, and treatment;
  • Psychoeducation regarding the identification and self-management of the prescribed medication regimen, with
  • Intensive case management;
    • Assessment
    • Planning
    • Linkage and referral to paid and natural supports
    • Monitoring and follow-up
    • Arranging for psychological and psychiatric evaluations; and
    • Crisis management, including crisis planning and prevention
